USF 499-A Exemption
The Federal Communications Commission (FCC) rules require HyperCube Telecom, LLC (HyperCube) to obtain a valid Federal Universal Fund Surcharge (FUSF) Fee Wavier Certification form every year from each customer.
To request a FUSF Fee waiver, please complete, sign and submit the Federal Universal Service Fund Surcharge/Fee Waiver Certification form (attached below) to HyperCube using one of the following methods:
